Arthur introduced his son - Ravi Retnakaran. Ravi is a Professor of Medicine at the University of Toronto.
 
Type 2 diabetes affects about 1 in 7 Canadians and can cause serious long-term complications including heart attack, stroke, kidney failure, blindness, nerve damage, and amputations. Because moderately high blood sugar often produces no symptoms, diabetes can develop for years before someone realizes they have it.
 
Ravi explains that type 2 diabetes results from a combination of genetics and environmental factors, particularly increasing obesity and abdominal fat. Excess abdominal fat can make the body resistant to insulin. However, insulin resistance alone does not necessarily cause diabetes. The critical issue is whether the pancreas's beta cells can produce enough insulin to compensate. Genetics play an important role in determining how well these beta cells function.
 
Healthy eating, exercise, and weight loss remain important because they reduce insulin resistance and take pressure off the pancreas. However, type 2 diabetes is generally progressive, and lifestyle changes cannot necessarily correct the underlying loss of beta-cell function. Many people eventually require medication despite making appropriate lifestyle changes.
 
A major part of the presentation focuses on GLP-1 medications, including Ozempic. GLP-1 is a natural gut hormone that helps increase insulin release, suppresses glucagon, slows digestion, reduces appetite, and promotes feelings of fullness. Drugs such as Ozempic imitate this hormone but have been designed to remain active much longer in the body.
 
These medications can be very effective for controlling diabetes and producing significant weight loss. By reducing weight and insulin resistance, they may potentially delay or prevent type 2 diabetes in some people who are genetically susceptible. However, the speaker stresses that Ozempic is not appropriate for everyone, has risks and side effects, and should be used under medical supervision.
 
Ravi also notes that people commonly regain weight after stopping GLP-1 medications because the drugs do not permanently change the body's underlying biology. It is still uncertain whether the best approach will ultimately be lifelong treatment, intermittent treatment, or medication combined with long-term lifestyle changes.
 
Main message: Type 2 diabetes is not simply the result of poor lifestyle choices. It develops through a complex interaction of genetics, insulin resistance, body weight, and declining pancreatic beta-cell function. Lifestyle remains essential, but newer GLP-1 medications such as Ozempic may significantly change how diabetes and obesity are treated and possibly prevented in the future.
